$500 Scholarship Application

Dear Student,

Please submit your application for scholarship with the following information.

Eligibility:

• Apply during senior year in High School or year of graduation.

• Enroll the same year student graduates from High School

• Submit essay

• Deadline: Spring Classes last week of May, Fall Classes first week of September, Winter classes last week of December. Please call for exact deadline dates.

Essay Requirements:

• 750 words. 2 pages double spaced

• Content – History, opportunity, and reason for pursing massage therapy as a career.

• Cover sheet: Name, address, phone #, high school name, year to graduate.

• Submit to: Scholarship application, ACMS, 100 South Main St., Crown Point, IN 46307

At least one student from each high school will be selected. The scholarship will be applied when student enrolls and completes all course requirements. Each student will be contacted before orientation day or 1st day of class.
